Public Health Department, Dyer County is a local health department that provides a variety of health services to keep people healthy and to get them well when they are sick.

Provider Description

Services often include:

  • Child Health: physical exams and screening tests
  • Immunizations
  • Primary Care: diagnosing and treating acute and chronic illnesses
  • Family Planning
  • STD Control 
  • Tuberculosis Control
  • Nutrition
  • Women, Infants, and Children (WIC): supplemental food program that provides nutrition assessment, education, and nutritious foods at no cost to pregnant, breastfeeding and postpartum women, infants, and children (up to age 5) at nutritional risk. 

CHANT Program:  care coordination of the following 3 programs

  1. Children's Special Services (CSS): assists with medical treatment for children to age 21 with special medical needs and the family can't financially pay for care
  2. HUG (Help Us Grow): home visits to families whose infants may be at risk for medical or developmental concerns
  3. TennCare Outreach: providing information, assisting with the application process, education concerning proper utilization of the managed care system, assisting in obtaining referrals and prior authorization for services, and assistance in locating providers

Hours: Monday-Friday, 8:00 a.m. to 4:30 p.m.
